Please ensure you have the following before you begin filling the form:

Completed Leader's recommendation & Member's recommendation forms from two different persons. (After the Leader and Member have each submitted their recommendations using these links, you will be emailed both recommendations. Please follow the instructions on the email to save and attach them to this online form).

2. Church Membership application: Water Baptism certificate.

3. Church Membership application (Transfer) only: Letter of Transfer from your previous church.

4. Completed both the Introduction to the Trinity Family course and a Discipleship Tool: New Life.
